ABOUT THE ROLE: Lead ongoing implementation and oversight of all Youth Coordinated Entry System (CES) activities in SPA 5.
Bring together key partners; facilitate regular Youth CES Care Coordination Meetings; collect and analyze housing assessment and placement data; train staff on Youth CES protocols; identify and communicate barriers; and report progress and lessons learned to community stakeholders.
Ensure consistent tracking and targeting of efforts and processes to fully meet Youth CES implementation goals.
WHAT YOU'LL DO: Work closely with homeless service providers, physical and mental health agencies, youth service providers, law enforcement, funders, and other stakeholders to better assess, assist, and house young people experiencing homelessness in SPA 5.
Provide thorough information to Youth CES staff at LAHSA and other participating agencies.
Ensure proper utilization of assessment and housing placement tools.
Facilitate Youth CES provider team meetings and bi-weekly case conferencing.
Monitor program data, documentation of services, and progress towards outcomes utilizing HMIS consistently.
Run monthly cross-system reports and collate youth outcomes for easy report-out.
Complete reports and documentation as required by Safe Place for Youth, United Way, LAHSA, and other funders in compliance with all reporting requirements.
Identify relevant interventions to resolve homelessness for program participants community wide.
Oversee contract execution pertaining to youth in SPA 5, ensure proper operations of all contracts and performance targets are met.
Train SPY staff and SPA 5 staff on Youth CES principles, conduct regular outreach to engage new and regular local partners in understanding Youth CES.
Establish and maintain effective working relationships and referral networks with partner agencies, housing providers, and related resources to promote access to available services.
Supervise Youth CES Matcher in SPA 5, oversees the process of matching youth experiencing homelessness to viable housing resources, be it transitional or permanent.
Supervise Youth CES Peer Navigator in SPA 5, oversee advocacy outreach, regular peer outreach, conduct assessments.
Supervise Problem Solving Specialists at the Access Center, ensuring all deliverables and contractual obligations are met Maintain regular communication with SPA 5 CES staff for the Single Adult and Family systems including SPA 5 CES Director, All Populations, and with YCES RCs in other SPAs, and their respective sponsoring agencies.
Coordinate regional outreach efforts and establish local access points.
Ensure linkage of participants to housing navigators and housing retention staff.
Other duties as assigned to support the mission of the program and organization.
Requirements SKILLS YOU'LL NEED: Knowledge in policies, systems, and local programs that impact homeless persons and the related public funding sources, regulations, requirements, and procedures as well as best practice models including: Housing First, Mental Health Recovery, Motivational Interviewing, Harm Reduction, and Critical Time Intervention Data and project management skill set Professional experience working within and leading multidisciplinary teams BACKGROUND YOU'LL NEED: Bachelor's Degree in Human Services, Social Work, Psychology, Sociology or related field is preferred .
2-3 years of experience in field based case management, counseling or related experience is required .
2-3 years of experience providing direct services to high-risk youth including counseling and advocacy is required.
1–3 years of experience working with youth experiencing or at risk of homelessness is required.
2-3 years of experience in a supervisory role is required .
2+ years of direct experience with individuals experiencing homelessness and housing instability and/or experience assisting low and very-low income individuals access affordable housing Bachelor's degree in human services or related field required Must have reliable transportation, valid California driver's license and proof of current insurance WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Prolonged periods of sitting at a desk and working on a computer.
Changes in the environment, such as the office or outdoors.
Physical effort/lifting, such as sedentary- up to 15-20 pounds at times.
SPECIAL CONDITIONS: Mandated Reporting Requirements: The person holding this position is considered a 'mandated reporter' under the California Child Abuse and Neglect Reporting Act and is required to comply with the requirements set forth in CSU Executive Order 1083 Revised July 21, 2017 as a condition of employment.
COVID-19 Vaccination Requirements: Effective August 9, 2021, the State Health Officer has mandated that all workers, paid or unpaid, in homeless services organizations must provide proof of vaccination.
If eligible for a booster shot, proof of booster is also required.
If not fully vaccinated, weekly COVID-19 testing is necessary depending on the program .

Driver Positions: Applicants are required to provide a copy of their driving record from the Department of Motor Vehicles, along with a copy of their valid driver's license and proof of insurance.

Examples of an unsatisfactory driving record include: Suspended or revoked license Three or more moving violations in the past 36 months Any violations, including drugs, alcohol, controlled substances, within the past 24 months Reckless driving, including hit and runs, within the past 24 months At fault accidents, resulting in fatality or serious injury, within the past 5 years The motor vehicle record will need to include a review of all states listed on the individual's employment application and resume.
