Collaborates with clinical and non-clinical team members to ensure a culture of support is reflected in members' treatment.This role supports Oneida or Onondaga County and surrounding counties across New York State with a Hybrid work model.
Applicant must live within the health plan service areas Onondaga or surrounding counties and will support a hybrid work model with remote work and travel to plan member's residences.Performance ResponsibilitiesTravels to members' homes to evaluate their plan of care and ongoing needs for community health services and support at least every six months or as directed by Clinical Team.Collaborates with clinical and non-clinical team members to ensure a culture of support in reviewing and updating the members' care plans.Provides support to health plan members, including orientation to the health plan, and the review of members' care plan including services and supports.Reports on members' current level of care needs in the community to clinical team members.Refers appropriate questions and issues to clinical and non-clinical team members.Conducts outreach to members who are missing monthly contacts.Assist with entitlement initiatives such as Medicaid benefits, spend-down, heating and food stamp assistance, and housing.Communicate with internal and external stakeholders in a timely and courteous manner.Participate in departmental meetings and attend all scheduled staff meetings and staff training as applicable.Perform related duties as required.RequirementsAssociates degree in human services, Licensed Practical Nurse, or equivalent of 1-3 years of relevant experience, with high school diploma or equivalency required.Strong computer literacy and comfort, ability, and desire to advance technically and understand industry standards.Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.Proficiency with computer systems, working knowledge of Microsoft Office, Internet, and various software applications.Valid NYS driving license required with own reliable transportation required.Physical RequirementsAbility to sit or stand at a desk 90% of the day.Travel throughout NYS.Visual/hearing ability sufficient to comprehend written or verbal communication.Ability to express self verbally and in writing.Ability to use telephone and communicate over the phone.Frequently uses a laptop computer to enter orders and obtain product information.Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids.Competitive Salary (This position is an S01 exempt position with a min-max rate of: $58,500-$67,718/annually) with:401K with generous Employer match.On-Site Gym (free for all employees).Potential Hybrid Position (Partial Work From Home).Tuition Reimbursement.Partially funded HSA.Employee Recognition Platform.Paid Time Off, Holidays, Sick and Extended Sick Leave.Short/Long term Disability.Employee Assistance Program (EAP).Much More!About Nascentia Health:Nascentia Health is leading the way in home care, post acute care and long-term community health.
